Parisa Farvid – Safety Culture Recognition

April 1, 2025: For going the extra mile by checking for and reporting swollen batteries during laptop inventory scans.

Marc Helou – Safety Spot Award

September 18, 2024: Marc has provided extensive support within the Computing Sciences Area, as well as to the overall LBNL community, to establish a safe process for the handling of lithium ion batteries. His contributions include development and communication of training materials, education of safe handling practices for both end users and support staff, removal of swollen batteries so they can safely be disposed of, and hazard evaluation of laptops and cell phones. He will continue to provide support until the Lab develops a formal policy on this matter.

Parisa Farvid – Safety Spot Award

September 18, 2024: Parisa provided extensive support to this year’s division self assessment on computer room access vs WPC authorization. Parisa played a key role in the data gathering for this assessment as well as in processing changes based on the corrective actions identified throughout the assessment.

Marc Helou – Safety Culture Recognition

August 8, 2024: Thanks for partnering with me on the li-ion battery safety presentation during CS Week! Your high level of expertise in this department is a strong asset towards improving the safety culture both within CS Area and across the Lab as whole.

Keith Beattie and Dan Gunter – Safety Spot Award

July 29, 2024: Two of the group leads in the AMCR Division recently reported safety concerns related to the use and transportation of hazardous chemicals from UCB to LBNL. Their willingness to come forth with these concerns allowed us to collaborate with the respective organization’s EHS divisions to resolve the unsafe behaviors and ensure the safety of all parties involved.

Erhan Saglamyurek – Safety Spot Award

November 22, 2023: As the supervisor of a staff member who was involved in a recent safety incident, Erhan provided an exemplary response both in the reporting and investigation phases of the incident. His actions allowed us to quickly respond to ensure the injured person received treatment, as well as to review the incident and take corrective action to prevent further occurrences.

Stefan Lasiewski – Safety Spot Award

March 24, 2023: Over the past two months Stefan has reported multiple hazards on the LBNL roadways and exterior staircases ranging from lighting issues to potholes. His prompt identification and reporting of these hazards allows us to implement corrective actions before any injuries or near misses occur. This is has been especially critical in recent months as increased numbers of staff and visitors come onsite, many of whom have never been to LBNL before and are unfamiliar with our terrain. Stefan is a strong asset to our safety culture.

Nick Balthaser – Hero Card

For efforts to maintain compliance with the SJHA process.

Stefan Lasiewski – Hero Card

For reporting exterior lighting hazards around B59.

Lipi Gupta – Hero Card

Moving fallen umbrellas on B59 patio to prevent and accident during the Perlmutter Pizza Party.

Jean Sexton – Safety Spot Award

June 16, 2022: Jean identified a significant slip/fall hazard in the 50 Complex in which someone had disposed of a partially full cup of tea in the compost bin and it leaked out all over the hardwood floor. An increased number of staff, including new summer students, have recently returned to the Lab and add the spill was located in an area that posed risk of blind corner. Someone could have easily slipped and sustained a serious fall injury had Jean not quickly identified and reported this hazard. Jean went above and beyond by assisting with the clean up of this spill in order to expedite the removal of this hazard.

Jeff Grounds – Safety Spot Award

June 16, 2022: Jeff is always a strong advocate for safety within NERSC, but in recent months he has made significant contributions as the Computing Sciences Area’s Electrical Safety Officer and the Building Manager of Shyh Wang Hall. Our FY22 Division Self Assessment identified gaps within the on-the-job training records for multimeter use within the NERSC and ESnet divisions and Jeff played a key role in the effort to bring us up to compliance. Jeff provided on-the-job training for safe multimeter use for both onsite and remote staff across NERSC and ESnet, to ensure that those authorized to perform this work know how to safely and effectively use a multimeter. Jeff has worked tirelessly throughout the pandemic to ensure that Building 59 (Shyh Wang Hall) is safe for its occupants and when it was discovered the water sources where missed in the lab-wide testing effort, Jeff was quick to respond. Jeff coordinated with the FAM and DSC to make arrangements for temporary water sources to be provided, while we awaited the results of the water testing. Thanks to Jeff we have been able to maintain a safe and comfortable working environment throughout this challenging time.

Jeff Grounds and Ernie Jew – Safety Spot Award

April 18, 2022: On Wednesday, January 26, 2022 the Alvarez Olympus cabinet was being installed next to an existing cabinet. Jeff Grounds conducted the LOTO safety briefing for the LBNL and HPE workers. During this meeting HPE did not bring it to anyone’s attention that the side panel would need to be removed from the existing cabinet and LBNL staff, include Jeff Grounds, was unaware that this would be required in the days work. Later on Ernest Jew observed that the HPE subcontractors had removed the side panel from the existing cabinet, exposing the 480V power connections. Since the cabinet was still powered on and in use, there was a potential for the removal of the side panel to result in a severe injury or death. Jeff and Ernest quickly took action to stop work, cordon off the area, power down the cabinet, and perform the necessary LOTO prior to the resumption of work.

Devin Baldwin – Safety Spot Award

August 3, 2021: The ashtray outside B59 smoked up while a group of interns was outside for some training. Devin noticed a suspicious smell and discovered that there was smoke coming out of the ashtray. He went to investigate and it was just active smoke, no fire. Devin put it out with water and created a video to disseminate this lesson to the rest of the team. His quick response prevented a potential wildfire from spreading onsite.

Owen James – Safety Spot Award

August 3, 2021: Owen displayed exceptional leadership in his response to a very serious health and safety incident in Building 59. Owen was quick and efficient in his response to the event and handled the situation with a high level of professionalism. Owen displayed a clear understanding of how to respond in an emergency, as well as the upmost concern for the well-being of his fellow colleague.
